f)  Operating System and Service Pack Number

g)  Esri Software and Version

h)  If you are a student user, please provide the contact information (full name, email and office phone number) of your supervisor in charge of the software.

i)  Description of problems and error or warning messages in details (if possible, please include screenshots of the error message)

j)  Steps or procedures performed leading to the error.
